i OX tiv 539 5. Scope of Work. Contractor agrees to provide on-site dental services to Medicaid/HealthChoice eligible or indigent children and related professional duties 32 hours per week. 6. Payment for Work. After the independent Contractor completes the services under terms of this Agreement and submits an Invoice on a monthly basis the LCPHD will pay the Contractor $ 75.00 per hour for services rendered. In addition, LCPHD will divide with the Contractor those monies earned over and above our monthly cost of $24,780.00 on a 50% / 50% basis. 7. Modification or Amendment. No amendment, change or modification of this Agreement shall be valid unless in writing and signed by the parties hereto. 8. Licenses/Certifications. Contractor shall be responsible for obtaining whatever licenses, permits, and/or certifications are necessary under applicable federal, state and local laws to fulfill his service obligations under the terms of the Agreement. 9. Tax Payments. Contractor understands that LCPFID will not contribute to Medicare, Social Security, unemployment insurance or any other required employment taxes, nor will LCPFID withhold federal or state income taxes from compensation paid to Contractor. Contractor understands he will be responsible for paying 100 percent of all applicable employment and income taxes. 10. Income Reporting. Contractor agrees that he will repot to federal, state and local government agencies, as required by law that he has been paid by LCPHD certain sums for services rendered under the terms of this Agreement.
