UUV~I Z G ME 5 ~ 530 LEE COUNTY PUBLIC HEALTH DEPARTMENT INDEPENDENT CONTRACTOR AGREEMENT This Agreement is entered into this Day Between (Contractor) who resides at and the Lee County Public Health Department LCPHD whose address is 106 Hillcrest Drive, Sanford, N.C. 27330. 1. Services. Subject to the terms and conditions of the Agreement, the LCPHD hereby engages the Contractor as an independent contractor to perform the services set forth herein, and the Contractor hereby accepts such engagement, 2. Period of Performance. The term of Agreement shall be from to 3. Termination. Either party may terminate this Contract without cause by giving written notice to the _ other at least thirty (30) calendar days prior to effective date of termination stated in the - notice. If Contractor fails to fulfill his or her obligations, the LCPF D may terminate this Contract by giving written notice to the Contractor at least seen (7) calendar days before the effective date of termination stated in the notice. The notice shall state the circumstances of the alleged breach and may state a period during which the alleged breach may be cured, which cure shall be subject to approval by the LCPHD. 4. Obligations in the Event of Termination. Upon termination of this Agreement, all finished or unfinished documents, data, studies and reports prepared by the Contractor pursuant to this Agreement shall become the property of the LCPHD. The LCPHD shall promptly pay the Contractor for all services performed to the effective date of termination, subject to the indemnification provision of this Agreement hereof and subject to offset sums due the Contractor against sums owed by the Contractor to the LCPHD.
