UUV~I Z G ME 5 ~ <br />530 <br />LEE COUNTY PUBLIC HEALTH DEPARTMENT <br />INDEPENDENT CONTRACTOR AGREEMENT <br />This Agreement is entered into this Day <br />Between (Contractor) who resides at <br />and the Lee County Public Health Department LCPHD whose address is <br />106 Hillcrest Drive, Sanford, N.C. 27330. <br />1. Services. <br />Subject to the terms and conditions of the Agreement, the LCPHD hereby engages <br />the Contractor as an independent contractor to perform the services set forth herein, and <br />the Contractor hereby accepts such engagement, <br />2. Period of Performance. <br />The term of Agreement shall be from to <br />3. Termination. <br />Either party may terminate this Contract without cause by giving written notice to the _ <br />other at least thirty (30) calendar days prior to effective date of termination stated in the - <br />notice. <br />If Contractor fails to fulfill his or her obligations, the LCPF D may terminate this <br />Contract by giving written notice to the Contractor at least seen (7) calendar days before <br />the effective date of termination stated in the notice. The notice shall state the <br />circumstances of the alleged breach and may state a period during which the alleged <br />breach may be cured, which cure shall be subject to approval by the LCPHD. <br />4. Obligations in the Event of Termination. <br />Upon termination of this Agreement, all finished or unfinished documents, data, <br />studies and reports prepared by the Contractor pursuant to this Agreement shall become <br />the property of the LCPHD. <br />The LCPHD shall promptly pay the Contractor for all services performed to the <br />effective date of termination, subject to the indemnification provision of this Agreement <br />hereof and subject to offset sums due the Contractor against sums owed by the Contractor <br />to the LCPHD. <br />
