B K 00025 PG North Carolina Lee County SERVICE CONTRACT This Contract is made, and entered into this the -- �� day of , 2013 by and between Lee County, a public body corporate of the State of North Car ' a, (hereinafter referred to as "the County ") and Harold Morcombe (hereinafter referred to as "Instructor "). 1. Services to be provided and agreed charges Instructor shall provide to Lee County Aquatic staff (hereinafter referred to as "student") lifeguarding instruction at the CPRO level for professional rescuers and renewal CPR classes (hereinafter collectively referred to as "Services "). The Lifeguarding class shall include CPR and First Aid. Services shall be provided at the Bob E. Hales Center for the classroom portion of the class and O. T. Sloan Pool for water safety instruction. In consideration of the services provided by Instructor, County shall pay to Instructor the following amounts: • CPR, renewal class only (no materials included): @ $82.50 per student • Full Lifeguard Class (includes CPR, First Aid and all materials): @ $300.00 per student • Materials, as necessary: • Masks: @ $10.00 per mask • CPR Books: @ $5.95 per book • Lifeguard Books: @ $32.00 per book The class size is determined by the Instructor. The classroom portion of the Lifeguarding class and CPR renewal class shall meet May 13, 14 and 15, 2013 at the Bob E. Hales Center. The water safety instruction portion of the class shall meet May 21, 22 and 23, 2013 at O.T. Sloan Pool. 2. Term of Contract The term of this contract for services is from May 6, 2013 to June 30, 2013. This contract is subject to the availability of funds and may be terminated at any time if such funds become unavailable. 3. Payment to Contractor County agrees to pay at the rates specified for Services satisfactorily performed in accordance with this contract. Unless otherwise specified, Instructor shall submit an itemized invoice to County upon the completion of the entire set of classes,
