BOOK Gi PGGF 647 ATTACHMENT A SCOPE OF SERVICES It is mutually agreed that the Scope of Services are as follows: 1. Contractor is to transport individuals scheduled through the COLTS program, with 48 hours notice whenever possible, at the following rates: $2.60 Drop Fee (when meter starts) $2.40 per mile $25.00 per hour wait time Maximum of four (4) individuals per vehicle per trip. This agreement applies only to those trips which the Contractor undertakes to transport passengers scheduled through the COLTS staff. Payment for trips scheduled by individual agencies or individual persons shall not be covered or reimbursed under this contract. 2. No-shows will be reported to the COLTS office immediately. 3. Contractor shall maintain invoices and trip logs for all work performed pursuant to this contract. Contractor will invoice COLTS on a monthly basis for services rendered and include the passengers name, address, and total number of miles charged per trip per passenger. The COLTS coordinator will verify invoices and submit them to the Director of the Department of Senior Services, who upon approval shall submit them to the Lee County Finance Office for payment. 4. In accordance to the Federal Transit Administration (FTA) drug and alcohol testing policy and regulations, Contractor agrees to provide for all drivers, pre-employment drug and alcohol testing, random drug and alcohol testing, post-accident drug and alcohol testing and reasonable suspension testing. Reports as requested by the FTA will be made available to COLTS for submission to the FTA for audit purposes and verification of procedures. 5. All information regarding a client of COLTS shall remain confidential and never released to any party without authorization of the individual or the COLTS coordinator or the Director of the Department of Senior Services.
