BOOK Gi PGGF 647 <br />ATTACHMENT A <br />SCOPE OF SERVICES <br />It is mutually agreed that the Scope of Services are as follows: <br />1. Contractor is to transport individuals scheduled through the COLTS <br />program, with 48 hours notice whenever possible, at the following <br />rates: <br />$2.60 Drop Fee (when meter starts) <br />$2.40 per mile <br />$25.00 per hour wait time <br />Maximum of four (4) individuals per vehicle per trip. <br />This agreement applies only to those trips which the Contractor undertakes to <br />transport passengers scheduled through the COLTS staff. Payment for trips <br />scheduled by individual agencies or individual persons shall not be covered or <br />reimbursed under this contract. <br />2. No-shows will be reported to the COLTS office immediately. <br />3. Contractor shall maintain invoices and trip logs for all work <br />performed pursuant to this contract. Contractor will invoice COLTS <br />on a monthly basis for services rendered and include the <br />passengers name, address, and total number of miles charged per <br />trip per passenger. The COLTS coordinator will verify invoices and <br />submit them to the Director of the Department of Senior Services, <br />who upon approval shall submit them to the Lee County Finance <br />Office for payment. <br />4. In accordance to the Federal Transit Administration (FTA) drug and <br />alcohol testing policy and regulations, Contractor agrees to provide <br />for all drivers, pre-employment drug and alcohol testing, random <br />drug and alcohol testing, post-accident drug and alcohol testing and <br />reasonable suspension testing. Reports as requested by the FTA <br />will be made available to COLTS for submission to the FTA for <br />audit purposes and verification of procedures. <br />5. All information regarding a client of COLTS shall remain <br />confidential and never released to any party without authorization of <br />the individual or the COLTS coordinator or the Director of the <br />Department of Senior Services. <br />
