0 0 <br />BOOK 21 PAGE 703 <br />STATE OF NORTH CAROLINA <br />COUNTY OF LEE <br />LEASE AGREEMENT <br />THIS LEASE AGREEMENT, hereinafter referred to as the "Lease", is made and entered <br />into this day of , 2007, between the COUNTY OF LEE, one of the counties of <br />the State of North Carolina and a body both politic and corporate under the laws of the State of <br />North Carolina, hereinafter referred to as "LANDLORD", AND AMISUB OF NORTH CAROLINA, <br />INC., a North Carolina corporation, with its principal office and place of business at 1135 <br />Carthage Street, Sanford, North Carolina, 27330, d/b/a CENTRAL CAROLINA HOSPITAL and <br />hereinafter referred to as "TENANT". and <br />WHEREAS, Landlord and Tenant entered into an emergency medical services <br />agreement as of 2007, pursuant to which Tenant will provide <br />emergency and non-emergency medical services within Lee County, North Carolina, and <br />WHEREAS, the Landlord is the owner of the premises hereinafter described and <br />proposes to lease same to Tenant upon the terms and conditions hereinafter set forth; and <br />WHEREAS, the Tenant is willing to lease said premises upon such terms and <br />conditions; <br />NOW, THEREFORE, in consideration of the respective agreements hereinafter <br />contained, the Landlord and Tenant agree as follows- <br />1 . LEASED PREMISES. The premises are described as a building of 7,280 rentable <br />square feet situated on the 1.14 acre tract located at 1218 Central Drive, Sanford, NC, <br />27330, ("Premises") and also identified as Lot No. 2, Central Carolina Professional Park, <br />Sanford, NC, and more particularly described in Exhibit "A" attached to this instrument <br />and by this reference made a part hereof. <br />2. TERM. <br />a) The term of this Lease shall be seven (7) years commencing on the 1s` day of <br />October, 2007 ("Commencement Date") and expiring on the 301h day of <br />September, 2014, unless sooner terminated as provided herein. <br />b) Should the Tenant hold over and remain in possession of the Premises after the <br />expiration of this Lease without the written consent of the Landlord, such <br />possession shall be as a month-to-month tenant. During such tenancy, monthly <br />rent shall be payable in an amount equal to the rent paid for the last month of the <br />term hereof plus fifteen percent (15%) of the monthly base rental amount. All <br />other terms and conditions of this Lease shall continue in full force and effect <br />during such tenancy, which shall be terminable by either party upon the delivery <br />of fifteen (15) days prior written notice. <br />c) Tenant shall vacate the Premises in the same good repair as received on the <br />Commencement Date, ordinary wear and tear excepted, and provided Tenant is <br />
