FIRST AMENDMENTTO SCHOOL NURSE FUNDING <br /> This First Amendment to the Lee County Health Department and Lee County Schools (the <br /> "Amendment"),is made effective August 1, 2022(the "Effective Date"),by and between the Lee County <br /> Health Department("LCHD")and Lee County Schools ("LCS"). <br /> WHEREAS,LCHD and LCS entered into that Activity 803 funding in the amount of 150,000 would pay <br /> for school nurses effective as of August 1,2022, (collectively referred to as the "Contract");and as <br /> amended by the First Amendment to School Nurse Funding Agreement(the "Amendment"),is made <br /> effective as of August 1, 2022(collectively referred to as the "Contract");and <br /> WHEREAS,anycapitalized term used, but not otherwise defined, herein shall have the same <br /> meaning ascribed to such term as set forth in the Contract;and <br /> WHEREAS,LCH and LCS have agreed to increase the funding of the Contract,as more particuladyset <br /> forth herein. <br /> NOWTHEREFORE,the Contract is hereby amended as follows: <br /> 1. Funding of Contract.The funding of the Contract is hereby increased$157,465 for school nurse <br /> funding,and thus now continues until May31, 2023. <br /> 2. A new subsection. P shall be added tothe Lee County Schools Section and incorporated into the <br /> terms of the Agreement as follows: <br /> P. Follow all requirements of the Activity 620 Agreement Addenda(attached).All required job <br /> descriptions and quarterly reports in the agreement addenda are due to the LCHD Nursing <br /> Supervisor by the l01h of the month following the ending quarter. For example: <br /> • June—September 2022;due by October l01h; <br /> • October—December 2022;due by January loth <br /> • January—March 2023;due by April l0ih <br /> • April—May 2023;due by June 1 Oth <br /> 3. Miscellaneous. Except as specifically amended herein, all terms and conditions of the <br /> Contract shall remain in full force and effect, except as otherwise amended in writing. This <br /> Amendment may be executed in two or more counterparts, each of which shall be deemed an <br /> original but all of which together shall constitute one and the same Amendment. The <br /> counterparts of this Amendment may be executed and delivered by facsimile or other <br /> electronic signature by any of the parties to any other party and the receiving party may rely <br /> on the receipt of such document so executed and delivered by facsimile or other electronic <br /> means as if the original had been received. <br />
