Please provide details of <br />all expenses including routine charges. These expenditures may include telephone, postage, salary, equipment purchases, <br />internet services etc. <br />All budgets must be approved by the Agency. <br />Grantee Name: The Enrichment Center <br />Narrative: <br />The grant funds will be used to assist Lee County's Medicare beneficiaries by offering additional counseling opportunities <br />during the Annual Enrollment Period of October 15- December 7, 2012. Carolyn Blackman, trained SHIIP Counselor, will <br />work 24 days for 4 hours per day at a rate of $12.00 per hour for a total of $1152. The category "Other" consists of <br />advertising in Boom! Magazine, handout items that will promote the SHIIP program at community events and health <br />fairs; and volunteer appreciation. Travel and training will include the 2012 SHIIP Coordinator's Conference and any <br />additional training that may be of use to the SHIIP program.
