BK:00025 PG:0096 <br />Attachment B <br />For the period 7/1 -2012 — 6/30/2013 <br />Statement of Work <br />Grantee: The Enrichment Center <br />This statement should be a short summary describing what the Grantee does and how the Grantee will use these funds. <br />The terns of the contract between the SHIIP office and the agencies require local programs meet these goals for <br />SFY2013. The uses of these funds are not limited to but MUST include the following activities: <br />1) Hold regular (weekly) counseling clinics during the period 10/2012 through 3/2013; <br />2) Coordinate a county volunteer recognition event during the period 9/1/2012 through 10/31/2012; <br />3) Expand Low Income Subsidy (LIS) outreach and enrollment in the county by participating in at least two (2) outreach <br />activities targeted to LIS populations; <br />4) Attendance at the SHIIP Coordinators' Training Conference on July 17 and 18, 2012 is mandatory for all <br />coordinators; <br />5) Must submit Client Counseling (NPR) forms in atimely manner to the SHIIP office in Raleigh or through the <br />SHIPTalk website; <br />6) Each SHIIP coordinating site must reach out to 50% of the county population for activity events; <br />7) Each SHIIP coordinating site must counsel 1.5 %) of their county's Medicare population; <br />8) Represent SHIIP at health fair events by utilizing local certified SHIIP counselors; and <br />9) Conduct at least one (1) Medicare education class for new to Medicare beneficiaries in your local county. <br />SCOPE OF WORK: <br />(Maximum 2 pages) <br />The Enrichment Center is the headquarters for Lee County Senior Services, Senior Games of Lee County, Veteran's <br />Services, and the County of Lee Transit System. Space is made available for the SHIIP trainings and counseling sessions. <br />Information sessions are held at the Center and in the community regarding the Medicare related choices including <br />Medicare Prescription Drug and the Medicare Advantage Plans that are available to beneficiaries. Free counseling <br />sessions are held at the Center and scheduled through the local SHIIP coordinator. These counseling clinics are by <br />appointment with the SHIIP Coordinator and volunteers are utilized as needed. The allocation will be used to enhance the <br />program through informational materials, travel and training reimbursement, advertising, and recognition of SHIIP <br />volunteers. Additional events are planned to promote and educate the public about the upcoming 2012 Annual Enrollment <br />period regarding Medicare Part D. To maintain awareness of the services offered, promotional items, brochures, and other <br />printed materials will be used during outreach opportunities such as Expos, community events, and health fairs. <br />Information explaining the requirements for the Low- Income Subsidy (LIS) will be distributed to members of the <br />community, including faith based and civic organizations, who may be able to assist SHIIP with reaching the remaining <br />Low- Income Subsidy (LIS) eligible beneficiaries in our county. An informational program titled "Happy 65" Birthday - <br />Medicare is next...now what ?" will be held quarterly to assist and inform upcoming Medicare beneficiaries about their <br />options and choices. <br />Page 10 of 15 <br />
