J <br /> STATE OF NORTH CAROLINA <br /> LEASE AGREEMENT <br /> COUNTY OF LEE <br /> THIS LEASE AGREEMENT, hereinafter referred to as the "Lease" is made and entered <br /> into this I St of day of October, 2021, (the "Effective Date") between the COUNTY OF LEE, one <br /> of the counties of the State of North Carolina and a body both politic and corporate under the <br /> laws of the State of North Carolina, hereinafter referred to as "LANDLORD", and FirstHealth of <br /> the Carolinas, Inc. a North Carolina non-profit corporation with its principal office and place of <br /> business at 155 Memorial Drive, Pinehurst, NC 28374 and hereinafter referred to as"TENANT"; <br /> and, <br /> WHEREAS, Landlord and Tenant entered into an emergency medical franchise <br /> agreement as of October 1, 2021 pursuant to which Tenant will provide emergency and non- <br /> emergency medical services within Lee County, North Carolina; and, <br /> WHEREAS, the Landlord is the owner of the premises herein described and proposes to <br /> lease same to Tenant upon the terms and conditions hereinafter set forth; and, <br /> WHEREAS, the Tenant is willing to lease said premises upon such terms and conditions; <br /> NOW, THEREFORE, in consideration of the respective agreements hereinafter <br /> contained,the Landlord and Tenant agree as follows: <br /> 1. LEASED PREMISES. The premises are described as a building of 7,820 rentable square <br /> feet situated on the 1.14-acre tract located at 1218 Central Drive, Sanford NC 27330 (the <br /> "Premises") and also identified as Lot No. 2, Central Carolina Professional Park, Sanford, <br /> NC. <br /> 2. TERM. <br /> a. The term of this Lease shall be for sixty days commencing on the 1St day of <br /> October,2021 ("Commencement Date") and expiring on the 30th day of <br /> November, 2021, unless sooner terminated as provided herein. <br /> b. Should the Tenant hold over and remain in possession of the Premises after the <br /> expiration of this Lease without the written consent of the Landlord, such <br /> possession shall be as a month-to-month tenant. During such tenancy, monthly <br /> rent shall be payable in an amount equal to the rent paid for the last month of the <br /> term hereof plus fifteen percent (15%) of the monthly base rental amount. All <br /> other terms and conditions of this Lease shall continue in full force and effect <br /> during such tenancy, which shall be terminable by either party upon the delivery <br /> of fifteen (15) days prior written notice. <br /> c. Tenant shall vacate the Premises in the same good repair as received on the <br /> Commencement Date, ordinary wear and tear excepted, and provided Tenant is <br />
