BK:00024 PG:0904 <br />MEMORANDUM OF UNDERSTANDING <br />BETWEEN <br />CUMBERLAND COUNTY HEALTH DEPARTMENT <br />AND <br />LEE COUNTY HEALTH DEPARTMENT <br />WHEREAS, the Cumberland County Health Department is the Lead agency for <br />the Community Transformation Grant (CTG) and provides guidance to Lee County <br />Health Department, and <br />WHEREAS, Lee County Health Department is responsible for CTG activities as <br />outlined in NC Division of Public Health Agreement Addendum 885 ( "AA 885'), <br />NOW THEREFORE, the Cumberland County Health Department, and Lee <br />County Health Department, hereby agree to this Memorandum of Understanding <br />( "MOU ") and the following responsibilities: <br />Responsibilities of Cumberland County Health Department <br />• Employ Lead Coordinator for CTG. This Lead Coordinator will also <br />serve as Tobacco Control Lead. <br />• Provide guidance on CTG and directives as accepted by the <br />Division of Public Health. <br />• Complete all financial reporting and monitoring as required by <br />Division of Public Health. <br />• Facilitate the hiring of a minimum of two full -time (2.0 FTE) CTG <br />Community Coordinators to serve the entire collaborative area, one <br />of which will be based in the lead agency. <br />• Facilitate partnership development, coalition building, community <br />organizing and partner communications. <br />• Provide monthly programmatic reports to the state. <br />• Update the agency's health directors on programmatic and fiscal <br />issues regularly. <br />• Build partnerships with the Quality Improvement Coaches based in <br />the regional AHEC Offices. <br />• Build relationships including representatives from AHEC, CCNC, <br />health departments, colleges /community colleges, affordable <br />housing, city or regional planning departments, schools, <br />PAGE 1 <br />
