tour~ 24 -4 667 <br />1.0 SCOPE OF SERVICES — APPLICATION PREPARATION <br />1.1 Attend NC Community Investment and Assistance (CI) workshop(s) concerning <br />the 2011 CDBG /SSH program. <br />1.2 Meet with Sanford /Lee County Community Development staff to discuss the <br />application process and identify roles and responsibilities. <br />1.3 Coordinate /attend meeting(s) of Sanford /Lee staff and Town of Broadway staff <br />(Housing Selection Committee) to discuss program guidelines, housing assistance <br />policy, and selection of applicants. <br />1.4 Provide information for the two required public hearings (Sanford /Lee staff to <br />advertise and conduct hearing). Attend public hearings if requested by staff. <br />1.5 Assist with Notice of Intent to Apply (CI) and applicant application form. <br />1.6 Prepare advertisement for applicants (Sanford /Lee staff to advertise). <br />1.7 Based on established rating /evaluation criteria, determine applicant priority for <br />eligibility and confirm selection with Sanford /Lee staff. Meet with selected <br />applicants to complete household site occupancy records. <br />1.8 If required by funding agency, complete work write -ups on selected houses. <br />1.9 If required by funding agency, complete environmental review record for selected <br />houses. <br />1.10 Complete all activities and furnish all required documentation to successfully <br />submit a FY 2011 CDBG SSH grant application on or before the deadline <br />established by Cl (March 30, 2012). <br />2.0 SERVICES PROVIDED BY THE COUNTY <br />2.1 Provide full information to the Consultant as to the requirements for the project. <br />2.2 Designate in writing a person to act as the County's representative with respect to <br />the work to be performed. Such person shall have complete authority to transmit <br />instructions, receive information, interpret, and define the County's policies. <br />2.3 Assist the Consultant by placing at his disposal all available information pertinent <br />to the project as may be required by the Consultant. <br />2.4 Guarantee access to and make all provisions for the Consultant to enter public and <br />private property as required for the Consultant to perform his services. <br />Agreement for Professional Services 2 <br />
