0 <br />BOOK li ~sE 365 <br />B. APPLICATION FOR REIMBURSEMENT <br />® 1. To be eligible for reimbursement all applications must be completed <br />and forwarded to the County Manager prior to registration and payment of the <br />courses. The cost of courses will be paid initially by the employee. <br />2. The employee shall prepare an Application for Educational Assistance <br />which may be obtained from the employee's immediate supervisor or from the <br />County Manager. <br />3. If the immediate supervisor recommends approval of the application, he shall <br />sign in the appropriate section and forward for the following required approvals: <br />(1) The department head <br />(2) The County Manager <br />4. The immediate supervisor shall be notified by the County Manager as to the <br />final disposition of the application. The immediate supervisor will, in turn, <br />notify the employee concerned. <br />0 C. LIMITATIONS <br />1. 100% of tuition fees for approved courses at accredited approved institutions <br />are reimbursable. <br />2. Employee will be reimbursed for 50% of the costs of textbooks, laboratory <br />supplies, and other required course material with the approval of the county <br />manager. <br />3. A passing grade of "C" or better is required for undergraduate work. <br />4. A passing grade of "B" or better is required for graduate work. <br />5. Termination of employment, for any reason, prior to completion of course will <br />make the employee ineligible for refund payment except in special cases as <br />authorized by the County Manager. <br />6. Reimbursement is limited to a maximum of $ 1,000.00 per fiscal year. <br />N~ <br />~11 <br />o~ <br />
