4.16.5.6.2 The special use permit application shall include complete copies of any required <br /> land disturbing permits as required by State and Federal agencies (such as Sedimentation and <br /> Erosion Control, Wetlands, etc.). Such copies shall be complete and include all required <br /> supporting documentation as required for said permits. <br /> 4.16.5.6.3 The special use permit application shall include complete copies of NCDOT <br /> driveway permits and/or other permits related to roadway access and construction. Such <br /> copies shall be complete and include all required supporting documentation as required for <br /> said permits. <br /> 4.16.5.6.4 The special use permit application shall also include a traffic impact study by a <br /> qualified consultant that will enable the Board of Adjustment to assess the impact of the <br /> proposed land use on the highway system when that system is at or near capacity or a <br /> safety problem exists. Its purpose is to ensure that the proposed land use does not adversely <br /> affect the highway network and to identify any traffic problems associated with access <br /> from the proposed site to the existing transportation network. The study shall also identify <br /> improvements to resolve traffic problems and present solutions that may be incorporated in <br /> the Site Plan. The study shall also include information sufficient to demonstrate <br /> compliance with the Road and Traffic Standards in this section. <br /> 4.16.5.6.5 If the proposed affected land is greater than ten (10) contiguous acres, the special <br /> use permit application shall include a Phase I Environmental Site Assessment of the proposed <br /> affected land completed not earlier than 12 months before the special use permit application <br /> date and prepared in accordance with the American Society for Testing and Materials. <br /> 4.16.5.6.6 The special use permit application shall include a major site plan as set forth in <br /> Appendix B-5 of this Ordinance. In addition to the site plan, the application shall also include <br /> an Operations Plan that details: <br /> • Details on how the mine will operate (where will initial excavation begin, where <br /> will overburden be placed, etc.) <br /> • Hours of operation <br /> • Number of employees, including details on shifts if applicable <br /> • Details on the function of each proposed structure or piece of equipment <br /> • Location of permanent roads (those to be used in excess of one year) and non- <br /> permanents roads <br /> 4.16.5.6.7 The special use permit application shall include a copy of the mining reclamation <br /> bond as required by NCGS 74-51 and 74-54 and as submitted to the NC Department of <br /> Environmental Quality (NCDEQ). <br /> 4.16.6 PERMITTED USES (UNDERLYING ZONING DISTRICTS) <br /> A Mining Special Use Overlay District will be an overlay on an underlying zoning district <br /> required specifically for mining and quarrying.
