Oil <br />LEE COUNTY <br />REQUEST FOR PROPOSAL <br />GRANT APPLICATION SERVICES <br />FY 2011 Community Development Program <br />Scattered Site Housing Application <br />Dear Offeror: <br />Lee County, North Carolina, anticipates receiving a $400,000 grant through the FY 2011 Small Cities <br />Community Development Block Grant (CDBG) Scattered Site Housing (SSH) Program. The County is <br />requesting proposals from qualified consulting firms to provide services necessary to complete an <br />application for the FY 2011 SSH Program. Service fees will be paid from Small Cities Community <br />Development Block Grant (CDBG) funds provided by the North Carolina Department of Commerce <br />(DOC) (100% of total program costs). <br />You are invited to submit a proposal to be received no later than 5 p.m., November 2, 2011 to Karen <br />Kennedy, PO Box 3729, Sanford, NC 27330 (mailing address) or 225 E. Weatherspoon Street <br />Sanford, NC 27330 (street address for delivery) phone: 919 - 775 -8238. (Please make sure you use <br />the correct address dependent upon your delivery mode.) <br />Two (2) copies of the proposal should be submitted in accordance with the following sections of this <br />Request for Proposal: <br />A. Problem Statement - <br />B. Scope of Work <br />C. Consultant Selection Procedures /Project Schedule <br />D. Fee Proposal <br />E. Proposal Content <br />F. Criteria for Awards /Evaluation <br />G. Federal and State Terms and Conditions <br />Lee County will enter into a contract with the Consultant whose proposal is determined to be the most <br />advantageous to the County, fee and other factors considered, contingent on the execution of a grant <br />agreement and funding approval. Factors to be considered, the method used in the evaluation of the <br />proposal and selection of the Consultant are set forth in Section F.
