13 Frc< <54 <br />3. Comply with all State licensing standards, all <br />applicable accrediting standards and any other <br />standards of criteria established by the Division <br />of Social Services to assure quality of service; <br />and <br />4. Restrict the use or disclosure of information <br />obtained in connection with the administration of <br />North Carolina's programs for the provision of <br />services concerning applicants for and recipients <br />of those services to purposes directly connected <br />with the administration of the service program; <br />and <br />5. Comply with the terms of Section 504 of the <br />Rehabilitation Act of 1973 and all requirements <br />imposed by or pursuant to the regulations of the <br />Department of Health, Education and Welfare issued <br />pursuant to that Section, which prohibit discrimination <br />against handicapped persons in employment and in the <br />operation of programs and activities receiving <br />Federal funds; and <br />6. Comply with Title VI and VII of the Civil Rights <br />Act of 1964 and all requirements imposed by or <br />pursuant to the regulations of the Department of <br />Health, Education and Welfare issued pursuant to <br />that Title; and <br />7. In the case of a private provider of medical or <br />remedial care or health related homemaker or chore <br />services, under Title XX, comply with the terms of <br />the Medicare-Medicaid Anti-Fraud and Abuse Act of <br />1977 (Public Law 95-142) and all requirements <br />imposed by or pursuant to the regulations of the <br />Department of Health, Education and Welfare issued <br />pursuant to that act which requires submission of <br />certain information concerning ownership or control, <br />and past business transactions; and <br />8. Accept fiscal responsibility for deviations from the <br />terms of this Contract as a result of acts of the <br />Provider or any of its officers, employees, agents <br />or representatives. <br />SECTION II: RESPONSIBILITY FOR LIABILITIES <br />Each party hereto agrees to be responsible for its own <br />liabilities and that of its officers, employees, agents, or <br />representatives arising out of this Contract. <br />SECTION III: PERSONNEL <br />A. Each applicant for employment must complete an application <br />for employment to be retained by the Provider. <br />B. Any program operated by or sub-contracted to an agency under <br />
