C49 <br />(e) List of material and equipment to be used. This should include <br />personal protective, equipment. <br />(f) Samples of documents /forms required by federal, sate and local <br />rules and regulations. <br />5.5 lVaste Identification: Fully describe the process by which accepted <br />materials are identified and analyzed on -site for proper segregation, <br />packaging and documentation prior to removal from site. <br />5.6 Packaging: Fully describe the process by which materials are removed <br />from collection areas and properly packaged for transportation. This <br />should include labeling and manifest preparation procedures. <br />5.7 Safety Program: Fully describe all safety considerations and <br />precautions and spill contingency plans with provisions for handling <br />excluded or non - acceptable wastes should they be received. <br />5.8 Ti-ansportation: Fully describe process by which wastes are to be <br />removed off -site and transported to their final destination. Please be <br />aware that all wastes must be transported off-site at the end of the <br />collection day or at a prior time if a quantity of waste capable of shipping <br />has been properly processed. <br />5.9 Disposal: Fully identify disposal methods and facilities for collected <br />wastes. <br />5.10 Reporting /Docmneatation: <br />(a) The Contractor shall turn over to the County a listing of all wastes <br />packaged for disposal prior to leaving the site. The Contractor <br />shall complete all manifests and shipping papers, and shall provide <br />such completed records to the County once receipt at a disposal <br />facility is complete. Final payment will not be made until the <br />County has received completed and signed manifests as well as <br />certificates of disposal or some other written documentation that <br />all wastes have been disposed of according to all federal, state and <br />local rules and regulations. <br />(b) Reports on any problems encountered during the collection event, <br />including but not limited to hazardous waste spills must be <br />submitted to the County within seven (7) days following said <br />collection event. <br />(c) Proposals should include copies of all Federal and State permits <br />required to cant' out the HHW collection event. Where permits <br />arc not immediately available, copies should be forwarded to the <br />
