pace ' 0_r 1S;OL- <br />Please read the Consolidated Agreement carefully and review the list below of changes from last year. <br />® Remember that the reporting of Local Appropriations by progranVactivity. is still necessary in the new <br />web-based expenditure reporting system. <br />Summary of Changes to the FY2009 Consolidated Agreement <br />11 <br />1) A sentence has been added in A.S. to allow DPH to submit the social security numbers of <br />all clients seen by local health departments to the Common Name Data Service to assure <br />de-duplication of clients in the new Health Information system (HIS). <br />2) The name of the Commission has been changed to the Commission for Public Health in <br />item A.7. <br />3) The name of the contracting entity to provide Board of Health training has been deleted <br />in item A.11. It now retlects that you will be notified by April 30 of the name of the <br />contractor for the upcoming fiscal year. <br />4) The language of A. 12 has been completely revised to reflect broader connectivity <br />concerns as well as I-IAN participation. <br />5) Item B.9. now includes language around the requirement for Health Departments <br />receiving more than $5,000,000 in Medicaid receipts each year to sign a Letter of <br />Attestation. <br />6) The wording in items B. 14.b. i. and ii. has been changed and specific wording related to <br />new PHP&R requirements and revised WIC requirements for prior approval have been <br />added. <br />7) Items C.3.e.l.iv and C.3.f.4. have been added; they relate to a new Line item (104) in the <br />Electronic Aid to Counties Website to capture the budget and funds expended for any <br />local match required for a program activity. <br />4) The MOE reporting requirement in G.22 has been deleted (note that compliance with the <br />MOE requirement remains.) <br />9) Dates have all been updated to appropriate references for FY09. <br />If you have questions or concerns, please contact vour Administrative Consultant.
