0 now : 13 994 a Stewart acoustical Consultants Post Office Box 30461 Raleigh, North Carolina 27622 Telephone (919) 781-8824 CONTRACT AND AGREEMENT FOR ' ACOUSTICAL CONSULTING SERVICES CLIENT: Address: SAMPLE Industrial Noise Control Architectural Acoustics Environmental Noise The above named client, hereinafter called "CLIENT," contracts with STEWART ACOUSTICAL CONSULTANTS, hereinafter called "CONSULTANT," to obtain acoustical consulting services within the following scope of work. SCOPE All work within this scope completed within one year of contract offer date will be charged according to the attached FEE SCHEDULE dated July 1, 1991. Additional work requested after one year from contract offer date will be charged according to the fee schedule then in effect. The current estimated cost of services described in the above scope of work is not more than $ CLIENT agrees to pay invoices of CONSULTANT not exceeding this estimate. This payment shall be complete compensation for services rendered under this contract unless amended. Final billing will be based on the actual time and expenses, and will not exceed the above amount without the concurrence of the CLIENT. Due to the nature of the work, unanticipated developments may make it advisable or necessary to exceed the above stated estimated cost to achieve the best possible result. If so, CONSULTANT will immediately advise CLIENT and seek concurrence or guidance. CLIENT has read and agrees to the attached "TERMS AND CONDITIONS OF CONTRACT AND AGREEMENT WITH STEWART ACOUSTICAL CONSULTANTS," revised July 15, 1991, which are a part of this contract and agreement.
