0 25? <br />STATE OF NORTH CAROLINA <br />CONTRACT <br />COUNTY OF LEE <br />THIS AGREEMENT made and entered into this day of <br />February, 1992, by and between the COUNTY OF LEE, a body both <br />politic and corporate under the laws of the State of North <br />Carolina, hereinafter referred to as COUNTY and SANDHILLS <br />FAMILY PRACTICE, P.A. a professional association created and <br />existing under the.laws of the State of North Carolina with <br />its principal office in Lee County, North Carolina; <br />hereinafter referred to as SFP; <br />WITNESSETH: <br />WHEREAS, the County operates the Lee County Jail and <br />desires to contract with SFP to provide medical services to <br />the inmates; and <br />WHEREAS, SFP is willing to provide such services and the <br />parties desire a written memorandum of their agreement; <br />NOW, THEREFORE, in consideration of the mutual <br />provisions herein contained the parties have agreed as <br />follows; <br />1. Medical Services. <br />SFP agrees to provide professional medical services for <br />the examination and treatment of inmates of the Lee County <br />Jail as follows: <br />a. SFP will conduct sick call for the inmates of the <br />Lee County Jail on the premises of said jail on three <br />separate days of each calendar week. Whenever possible such <br />visits will be made so that not more than two days elapse <br />between such visits. SFP shall notify the Sheriff's <br />Department well in advance of such visit and arrange a <br />mutually satisfactory time for such visit. <br />
