page 2 <br />02/15/06 020 <br />Please read the Consolidated Agreement carefully and review the list below of changes from last year. <br />Remember that the reporting of Local Appropriations by program /activity is still necessary in the new <br />web -based expenditure reporting system. <br />Summary of Changes to the FY2012 Consolidated Agreement <br />0 <br />1) Item A. 10 related to the Community Health Assessment and SOTCH reports has been <br />revised as follows: to add "at least" before every four years; to insert after "the <br />Community Health Assessment should be a collaborative effort" , the words "with local <br />partners such as hospitals", and 2 new sentences that reads "For action plans, the <br />agency shall include a minimum of two Healthy North Carolina 2020 objectives from <br />different focus areas. There are a total of 13 focus areas and 40 objectives within <br />Healthy NC 2020." <br />2) Item B. 1 1 is a new stipulation that "local health departments may not require a client to <br />present an identification that includes a picture of the client for at least immunizations, <br />pregnancy prevention, sexually transmitted disease and communicable disease <br />services ". <br />3) Item C.3.f. includes new language as follows: "or if the Agreement Addenda allows for <br />drawing down funds based on number of individuals screened or provided services, the <br />actual number screened or served multiplied by the per capita rate specified in the <br />Agreement Addenda ". <br />4) Item G. 10 no longer includes any reference to HSIS for billing Medicaid <br />5) Item K.2 no longer includes references to "subrecipient" or N.C.G.S. 143.6.2 when <br />referring to monitoring of local health departments; that language is retained in <br />paragraph 2 of that item which describes local health departments responsibilities for <br />monitoring entities to whom they give State or federal. <br />If you have questions or contains, please contact your Administrative Consultant. ]'banks for your <br />continued hard work for public health in North Carolina! <br />cc: Jeff Engel <br />Dennis Harrington <br />Chris Hoke <br />Judith McDermott <br />Sharon Smith <br />Administrative Consultants <br />PHMT <br />• <br />
