® fioo~ 15 FAI~_ 198 <br />SERVICE AGREEMENT <br />FOR PROVISION OF RESIDENTIAL SERVICES <br />TO JOHNSTON COUNTY <br />016 <br />This Agreement is made this the 7th day of -June , 1993, <br />between Lee County, North Carolina (hereinafter called the "Provider"), <br />and Johnston County, North Carolina (hereinafter called the <br />"Purchaser"). The Agreement will commence on July 1, 1993, and remain in <br />effect through June 30, 1994. <br />WITNESSETH: <br />WHEREAS, Provider operates a fully-licensed residential facility <br />that offers temporary shelter care, counseling, and referral services <br />for up to nine troubled youth; and <br />WHEREAS, Purchaser has an identified need to utilize residential <br />placement in serving troubled youth and desires to ensure the <br />availability of three bedspaces to use in fiscal year 1993-1994; <br />NOW, THEREFORE, in consideration of the written programmatic <br />agreement offered by the Provider and accepted by the Purchaser, <br />IT IS AGREED: <br />1. That the Purchaser will monetarily compensate the Provider at <br />the annual rate of $42,000 in order to guarantee the provision <br />of temporary shelter and related services for three (3) <br />bedspaces. <br />2. That the monetary compensation made to the Provider for <br />guaranteed services will begin on the 10th day of August in the <br />year 1993, with an initial payment of $21,000 and the remaining <br />$21,000 to be paid on the 10th day of January, 1994. <br />3. That the Purchaser and Provider will mutually abide by the <br />program-related responsibilities of each included in the <br />original written agreement offered by the Provider in the form <br />of a Memorandum of Understanding dated June 30, 1992. <br />IN WITNESS WHEREOF the parties hereto have on the day and year <br />first above written caused these conditions to be executed on their <br />behalf and in their organizational names respectively by their proper <br />officers hereunto duly authorized and their respective organizational <br />seals to be hereto attached by like authority. <br />
