0 0 <br />BPECP <br />BOOK 15 nArr 502 <br />2. Recordkeeping for educational training: <br />a. The dates of the training sessions <br />b. The contents or a summary of the training sessions. <br />c. The names and qualifications of persons conducting <br />the training. <br />d. The names and job titles of all persons attending <br />the sessions . <br />e. Training records shall be maintained for three (3) <br />years from the date on which the training occurred. <br />f. Records shall be made available upon request. <br />I. Medical Recordkeeping/exposure Incident <br />1. Medical records shall remain strictly confidential. <br />Medical records are not disclosed or reported without <br />expressed written consent of employee to any person <br />within or outside the workplace except as required by <br />law. <br />2. Medical records shall contain the following: <br />a. Name and Social Security Number <br />b. Copy of Hepatitis B vaccine status including dates <br />of all Hepatitis B vaccinations and medical records <br />relatively to the employees' ability to receive <br />vaccination. <br />c. Copy of all medical examinations, medical testing <br />and follow-up procedures related to occupational <br />exposure. <br />d. Copy of Health Care Professionals written opinion <br />e. Copy of information provided to Health Care <br />Professional. <br />3. Records to be kept for duration of employment plus <br />thirty (30) years. <br />V. COMPLIANCE DATES <br />A.
