Department Supervisor will complete Exposure Incident <br />Report and instruct personnel in procedures to follow <br />as a result of an actual or potential occupational <br />exposure. Supervisor shall complete all procedures <br />pursuant to Worker's Compensation policy. <br />3. The physician treating and evaluating an employee after <br />an exposure incident shall be furnished: <br />a. A copy of OSHA Standard 1910-1030; <br />b. Description of the duties performed at the time of <br />the exposure incident; <br />c. Documentation of the route of entry; <br />d. The results of the source blood test (if available); <br />e. All medical records relevant to the appropriate <br />treatment of the employee including vaccination <br />status. <br />4. The physician shall furnish a copy of the results of <br />the completed evaluation to the Department Head, who <br />will furnish a copy of the results to the employee, <br />within 15 working days of the completion of the <br />evaluation. The following information shall be <br />included: <br />a. Whether the employee has or should receive the <br />Hepatitis B vaccination series; <br />b. That the employee has been informed of the results <br />of the evaluation; <br />c. That the employee has been told about any medical <br />condition resulting from the exposure to blood or <br />other potentially infectious materials which require <br />further evaluation or treatment. <br />d. All other findings or diagnoses shall remain <br />confidential and shall not be included in the <br />written report. <br />e. Post-exposure prophylaxis, when medically indicated, <br />as recommended by the U. S. Public Health Service. <br />G. Communication of Hazards to Employees <br />10 <br />
