BOX 339, 400 ROBERSON ST. <br />CARRBORO, NORTH CAROLINA 27510 <br />LETTER OF AGREEMENT <br />BETWEEN <br />LEE COUNTY DEPARTMENT OF SOCIAL SERVICES <br />AND <br />ORANGE-CHATHAM COMPREHENSIVE HEALTH SERVICES, INC. <br />HAYWOOD MONCURE HEALTH CENTER <br />FOR <br />MEDICAID TRANSPORTATION SERVICES <br />TELEPHONE: <br />919-9428741 <br />This agreement entered this 7th day of February 1994 <br />by and between the Lee County Department of Social Services (DSS) <br />and Orange-Chatham Comprehensive Health Services, Inc. - Haywood <br />Moncure Health Center (OCCHS) is for the purpose of providing <br />health services transportation for medicaid eligible clients. <br />Whereas OCCHS provides transportation for Lee County residents <br />who are certified to be eligible for medicaid coverage; and <br />Whereas OCCHS serves many Lee County medicaid eligible clients <br />at its Haywood Moncure Health Center, and <br />Whereas the Lee County DSS desires to improve access to <br />quality primary care for its residents so as to reduce long term <br />cost to the County, <br />Now therefore the parties agree as follows: <br />1. OCCHS agrees to provide one way transportation between the <br />Lee County (Sanford) area and the Moncure Health Center for <br />$5.00. Two way (roundtrip) transportation will be charged <br />at $10.00. <br />2. OCCHS agrees to transport medicaid eligible women and their <br />children to receive health services regardless of whether <br />all members of the family are eligible for payment by DSS. <br />3. OCCHS will bill DSS monthly for transportation provided to <br />Lee County medicaid eligible residents at the rates indicated <br />above. A list of persons transported and their medicaid <br />numbers will be submitted with bills. <br />4. Lee County DSS agrees to pay OCCHS $5.00 one way and $10.00 <br />roundtrip for transporting Lee County medicaid eligible <br />clients to receive necessary health services. <br />O 1.'s <br />This agreement may be amended by written agreement of the <br />parties and is governed by the laws of the State of North Carolina. <br />
