We plan to serve the elderly of Curfi I Harnett, <br />Hoke, Moore and Robeson counties with a program called PACE (Program for I A 1 11 1 4 1 in clusive <br />elusive <br />Care for the Elderly). It is a unique program which provides a full spectrum of health care <br />services for the frail elderly certified to require nursing home level of care who desire to remain <br />at home for as long as possible. There are currently just two PACE programs operating in the <br />state of North Carolina and over 70 programs nationwide. To be eligible for PACE services, the <br />participant will need to be 55 years of age or older, live in our service area, meet the <br />requirements for nursing home eligibility and be able to live safely in the community with our <br />24/7 services. I am so excited to be involved in this project which will bring hope to seniors and <br />their families who wish to remain at home. This program is funded by Medicare, Medicaid and <br />private pay. For the over twelve years before coming to work at St. Joseph, I saw the need first <br />hand when working with the primary and specialty care practices with Cape Fear Valley Health <br />System. This new method of continuity of care for our elders will make a difference in their <br />lives and be more cost effective in a time where our nation needs to address the rising cost of <br />caring for our elderly. <br />• <br />
