X75 <br />AMENDMENT #1 <br />TO <br />AGREEMENT FOR PROFESSIONAL SERVICES <br />This Amendment is made to the original Agreement dated February 2, 2009 between L.E. <br />Wooten and Company d /b /a The Wooten Company (CONSULTANT) and the County of Lee <br />(CLIENT). <br />1. In accordance with Section 1 of The Agreement and Section Vl. Additional Services of <br />Exhibit A, thereto, this Amendment is for additional services (beyond those anticipated in <br />the original scope of work) to implement the CDBG -08 SSH program. Additional scope of <br />F,"0 3 <br />A. Extended efforts over a fifteen month period to assist first beneficiary identified for <br />relocation assistance. <br />B. Qualification of second beneficiary for relocation assistance including <br />environmental assessment, oversight of asbestos testing and clearance, <br />identification of replacement home, purchase negotiations, closing, demolition of <br />old home. <br />C. Conformity with increasing /changing compliance requirements from Division of <br />Community Assistance (now Community Investment and Assistance). Conformity <br />included development of language access plan and initial work on a new analysis <br />of impediments to fair housing form which has since been withdrawn by the State. <br />2. Total Compensation for additional services - $19,500. Sufficient grant funds remain to <br />cover the cost of additional services. <br />3.
