6. Disruptive Behavior: A passenger's right to transportation can be terminated <br />by COLTS due to misconduct of the passenger. Disruptive or abusive behavior <br />to other passengers or the driver will not be tolerated. EXAMPLES — <br />Profanity, possession of weapons, eating, drinking, smoking, loud talk or <br />singing, intoxication. <br />7. Operations during Inclement Weather — Please refer to COLTS Inclement <br />Weather Policy. <br />8. COLTS agrees to hold all information we may access about consumers or <br />former consumers confidential and will not divulge any information to any <br />unauthorized person. <br />B. The Agency LCSS shall: <br />1. Determine eligibility of clients to be transported and coordinate daily <br />requirements with transportation system dispatcher. <br />2. Provide COLTS (or its designated representative) the time and location of <br />pickups and discharges, the names, telephone numbers, and special needs (e.g. <br />vans with wheelchair lift) of the clients to be serviced no later than by 12:00 <br />noon two workdays before service is needed. <br />3. Provide COLTS with adequate notice of cancellation of prescheduled <br />transportation. " Adequate Notice" is defined as two (2) hours. <br />4. Reimburse the County for all transportation services rendered at the rates <br />hereinafter set out: <br />(a) Within the county transportation will be billed at the rate of $9.00 per <br />person per trip for Medical Services. Senior Services General/Nutrition <br />will be billed at $6.00 per person per trip. Out -of -County transportation <br />will be billed at the rate of $35.00 per trip <br />COLTS may adjust these rates at any time with a 30 day notice to <br />the Agency receiving this agreement. <br />6. Insure that no trips are taken involving transportation of Agency clients for <br />religious or political activities and/or events as these type trips are prohibited <br />by Federal regulations. <br />C. General Terms <br />1. This Agreement shall take effect as of July 1, 2017 and shall be effective <br />through the end of the County's fiscal year, June 30, 2018. This agreement will be <br />automatically renewed for successive one-year terms unless terminated in writing <br />
