FRANCHISE AGREEMENT THIS AGREEMENT, made and entered into this L&_ day of )t1Ve_MbC ~ 2002, by and between the COUNTY OF LEE, hereinafter referred to "County", and SANFORD-LEE COUNTY VOLUNTEER RESCUE SQUAD, INC., hereinafter referred to as "Provider." WITNESSETH: WHEREAS, the County has contracted with Central Carolina Hospital to provide emergency medical services and there are times when it receives calls for assistance when all units are occupied on other duties; and WHEREAS, Provider has trained personnel who are willing to volunteer their services to provide back-up emergency medical services as well as heavy rescue and extrication services; and WHEREAS, Provider has made application for a franchise to provide rescue services and back-up ambulance service to Central Carolina Hospital EMS; and WHEREAS, the County is willing to grant such franchise under the terms and conditions hereinafter set forth; and NOW, THEREFORE, in consideration of the premises and the mutual promises herein contained, the parties have agreed as follows: 1. The County hereby grants to Provider a franchise to operate a rescue service and to serve as a back-up ambulance service to Central Carolina Hospital EMS in Lee County in accordance with and while in compliance with the terms and conditions set forth in the following documents, all of which are incorporated herein by this reference, to wit: (a) Ambulance provider license application form (b) Lee County ordinance relating to the granting of franchises for ambulance services "Ordinance Book 1, Page 163, et. Seg. (c) All applicable State laws and regulations 2. This franchise is to begin on the date of the execution of this agreement by both parties and shall terminate on June 30, 2003.
