4.1 Truck/Trailer Certification: All trucks and/or trailers to be used in debris removal <br />operations must be measured to determine capacity and documented on a Truck <br />Certification Form by a monitor assigned by the County and working independently of <br />the Contractor. The Truck Certification Form must include, but not necessarily limited <br />to the following information: <br />• Capacity of hauling bed (CY) <br />• License plate number <br />• Truck identification number <br />• Brief physical description of the truck/trailer <br />• Photographs <br />• Driver information <br />• Owner information <br />• Name of person completing measurements and form <br />4.2 Load Tickets: A "load ticket" will be utilized as the primary debris tracking document. <br />The form of the ticket shall be approved by the County and shall include at a minimum <br />the following information: <br />• Ticket Number <br />• Contract Number <br />• Date <br />• Contractor Name <br />• Truck Number <br />• Truck Capacity <br />• Truck Driver Name <br />• Loading Location <br />• Loading Time <br />• Unloading Location <br />• Unloading Time <br />• Debris Classification <br />• Debris Quantity <br />• Monitor Name(s) and Signature(s) <br />Load tickets shall be accepted by the County as the certified, original source documents <br />to account for the measurement and accumulation of the volume/tonnage of debris <br />delivered and processed at the debris management site. The ticketing system will also <br />be used in the event of additional debris handling for volume reduction and/or the <br />possible requirement for a debris transfer station(s). These load tickets will also be <br />utilized for debris haul out for final disposal. These tickets will be the basis of any <br />electronic generated billing and/or report(s). <br />Page 7 <br />
