00052 <br />Page 2 of 4 <br />State appropriations have allocated approximately $420,000 for Fiscal Year 2017. The intent of this <br />Agreement Addendum is to make available approximately $20,000 dollars to qualified local health <br />departments based upon need, so that 100% of all local health departments can submit population health <br />and program service data to DPH. <br />II. Purpose: <br />As the Local Health Department has not yet transitioned to an electronic health record (EHR) pending <br />implementation of that component in the state's Health Information System (HIS), this Agreement <br />Addendum provides supplementary funding enabling the Local Health Department to acquire an EHR <br />system. <br />III. Scope of Work and Deliverables: <br />The Local Health Department shall: <br />1. Transition to an electronic health record system by May 31, 2017. <br />2. Apply funds to enhance public health reporting in one or more of the following ways: <br />a. Offset the initial purchase cost of a Centers for Medicare and Medicaid Services (CMS) <br />certified electronic health record <br />b. Support subscription services for use of electronic health record for e -prescribing <br />c. Support customization of an electronic health record to accommodate batch reporting to HIS, <br />and/or <br />d. Support imaging solutions to work in conjunction with an electronic health record. <br />3. Complete the Activity 113 Electronic Health Record Report for FYI (Attachment A) at the end of <br />the fiscal year in order to indicate how the funds were spent based upon locally determined needs. <br />4. In order to qualify for these funds, have in place a permanent or interim health director who either <br />meets the qualifications to serve as a county health director as required in NC GS 130A-40, or has an <br />approved exception and has been sworn in using the Oath of Office so that he or she can fulfill the <br />statutory functions assigned only to a local health director. In addition, if that health director has <br />never served in that role in North Carolina previously, that health director must participate in the <br />Orientation for New Local Health Directors coordinated by the North Carolina Association of Local <br />Health Directors. <br />IV. Performance Measures/Reporting Requirements: <br />1. By June 30, 2017, the Local Health Department shall submit the Activity 113 Electronic Health <br />Record Report for FYI (Attachment A) to the Local Technical Assistance & Training Branch <br />(LTAT), and report information for the FYI Service Period of October 1, 2016 — May 31, 2017. <br />Do not return Attachment A with the signed Agreement Addendum 113. This report shall <br />provide: <br />a. Information on how the funds were distributed <br />b. A copy of the Department of Natural and Cultural Resources approved Electronic Records <br />and Imaging Policy, and <br />c. A copy of the local board of health approved Electronic Health Records Policy. <br />2. The Local Health Department Batch Data Total Error Report (HIS Report 200, 201, or equivalent) <br />must be below 5% within six months of the go live date for the EHR. <br />Effective July 2015 <br />
