Number of unduplicated females receiving LARC insertions by income level (as a <br />percentage of the U.S. Department of Health and Human Services poverty guidelines) <br />using the following income level ranges: 100% and below; 101 % to 150%; 151 % to <br />200%; 201% to 250%; Over 250%, and Unknown/Not Reported. <br />As of November 1, 2016, Agreement Addendum Revision #1 adds Paragraph S, as follows: <br />5. Expand upon program activities under the evidence -based strategy or strategies the Local Health <br />Department has already chosen to implement in FYI 7. A detailed budget which outlines the <br />expenditures — for the expanded program activities and for the additional funds only — shall be <br />submitted to the DPH Program Contact for approval. <br />IV. Performance Measures/Reporting Requirements: <br />As of November 1, 2016, Agreement Addendum Revision #1 adds Subparagraph a to Paragraph 2, as <br />follows: <br />a. If the Local Health Department has chosen to implement Long Acting Reversible <br />Contraception (LARC) as an evidence -based strategy, it shall report the data described in <br />Section III Paragraph 4.a. <br />V.
