We appreciate the opportunity to work % ith you and Lee County in managing the medical needs of the inmates at the Lee County Jail. Our Health Services Agreement will soon renew in June, and we look forward to another year of providing services. In lieu of a contract Amendment, this correspondence serves to confirm the renewal and extension of our Health Services Agreement for the twelve-month period effective June 1, 2016, through May 31, 2017. As stated in our letter dated May 1, 2015, there will be no increase in the County's base compensations' fee or per diem rate for the 2016-2017 contract .,renewal period. If you have any concerns or wish to discuss renewal terms in further detail, then please contact me to advise. We will otherwise assume you and the County are in agreement, and all provisions of the contract will remain in full force and effect until such time as amended or modified in writing by the parties. A summary of the renewal pricing terms is provided as follows: Contract period: June 1, 2016, through May 31, 2017 Base annualized fee: $193,027.32 Base monthly fee: $16,085.61 Per diem greater than 125: $2.10 Annual outside cost pool limit: $50,000.00 Please keep this letter for your records and feel free to contact me if you should you have any questions.
