Brajer <br />Governor Secretary <br />Daniel Staley <br />Division Director <br />March 22, 2016 <br />MEMORANDUM <br />TO: Local Health Directors <br />FROM: Wendy Holmes, RN, Head W t f <br />Immunization Branch <br />SUBJECT: Funding Opportunity for Activity 715 Immunization Action Plan <br />The purpose of this memo is to make you aware of a funding opportunity available for your health department. <br />Federal immunization funds provided to your health department are designed to support immunization activities <br />outlined in the 715 Immunization Activity Plan. These activities assure that individuals are age appropriately <br />immunized, and reduce the occurrence of vaccine -preventable diseases. The time period for these funds being <br />made available to you gives your health department the option to receive the funds after March 31, 2016, or to <br />decline these funds. Acceptance of these funds will not change the scope of work and deliverables of the current <br />Agreement Addendum. The proposed allocation for your county is found in Attachment A. <br />Should you elect to receive these funds, they may be accessed through the WIRM in Activity 715 Immunization <br />Action Plan by April 1, 2016. No revision to your current Agreement Addenda is required in order for your health <br />department to receive these funds; however, they must be spent or encumbered by the end of the service period, <br />May 31, 2016. All expenditures must be reported by the liquidation date of June 30, 2016, and the funds must be <br />used in accordance with the guidance outlined in the SFY 15/16 Agreement Addendum for activity 715. <br />Please click on the following link and complete the short survey to indicate your agreement to receive or decline <br />these funds. https://www.surveymonkey.com/r/715IAP <br />The survey will remain active until March, 30, 2016. Funds will be allocated to those health departments that have <br />elected to receive them.
