<br />B. Reports to be provided during Period of Performance
<br />SUB -RECIPIENT must also provide a semi-annual summary (progress report); no later than July 15'h to the
<br />HSGP Grant Manager and/or Field Planner to ensure that the project deliverables are being met, and that each
<br />grant contract is operating within budget.
<br />C. Reports to be Provided at the Conclusion of Work (if applicable)
<br />i. Quarterly project progress reports.
<br />ii. Sub -Recipient involved legal action that pertains to Planning Training Exercise and Equipment
<br />purchased with HSGP ;
<br />iii. After action report from exercise,
<br />iv. Training course roster and description
<br />V. Any other documentation that would be pertinent.
<br />vi. Any invoices detailing the expenses associated with the project
<br />25. Lobbying Prohibition: The Sub -Recipient certifies, to the best of his or her knowledge and belief, that:
<br />A. No Federal appropriated funds have been paid or will be paid, by or on behalf of the undersigned, to any
<br />person or employee of any state or federal agency, a member of the N.C. General Assembly, a Member of
<br />Congress, an officer or employee of Congress, or an employee of a Member of Congress in connection with
<br />the awarding of any Federal contract, the making of any Federal Grant, the making of any Federal loan, the
<br />entering into of any cooperative agreement, and the extension, continuation, renewal, amendment, or
<br />modification of any Federal contract, grant, loan, or cooperative agreement.
<br />B. In any funds other than Federal appropriated funds have been paid or will be paid to any person for
<br />influencing or attempting to influence an officer or employee of any agency, a Member of Congress, an
<br />officer or employee of Congress, or an employee of a Member of Congress in connection with this Federal
<br />contract, grant, loan, or cooperative agreement, the undersigned shall complete and submit Standard Form -
<br />LLL, "Disclosure Form to Report Lobbying," in accordance with its instructions.
<br />C. The undersigned shall require that the language of this certification be included in the award documents for all
<br />sub -awards at all tiers (including subcontracts, sub grants, and contracts under grants, loans, and cooperative
<br />agreements) and that all sub -recipients shall certify and disclose accordingly.
<br />This certification is a material representative of fact upon which reliance was placed when this transaction
<br />was made or entered into. Submission of this certification is a prerequisite for making or entering into this
<br />transaction imposed by section 1352, title 31, U.S. Code. Any person who fails to file the required
<br />certification shall be subject to a civil penalty of not less than $10,000 and not more than $100,000 for each
<br />such failure.

<br />26. Assurance of Compliance with Title VI of the Civil Rights Act of 1964 During the performance of this
<br />contract, the contractor, for itself, its assignees and successors in interest (hereinafter referred to as the
<br />"contractor") agrees as follows:
<br />A. Compliance with Regulations: The contractor shall comply with the Regulations relative to nondiscrimination
<br />in Federally -Assisted Programs of the 2 C.F.R. 200 and North Carolina regulation as they may be amended
<br />from time to time, (hereinafter referred to as the Regulations), which are herein incorporated by reference and
<br />made a part of this contract.
<br />B. Nondiscrimination: The contractor, with regard to the work performed by it during the contract, shall not
<br />discriminate on the grounds of race, sex, or national origin in the selection and retention of subcontractors,
