B K 00026 PG Attachment B For the period 7/1/2014 — 6/30/2015 Statement of Work Grantee: The Enrichment Center This statement should be a short summary describing what the Grantee does and how the Grantee will use these funds. The terms of the contract between the SHIIP office and the agencies require local programs meet these goals for SFY The uses of these funds are not limited to but MUST include the following activities: 1) Conduct a minimum of seven (7) dedicated enrollment events (counseling clinics) - -one (1) must be dedicated to the disability population - -during the period 10/15/2014 through 12/7/2014 and conduct bi- weekly counseling clinics during the period 1/1/2015 through 2/14/2015; 2) Coordinate a county volunteer recognition event during the grant period; 3) Attend the SHIIP Coordinators' Training Conference July 17 & 18, 2014; this is mandatory for all coordinators; all new coordinators are required to attend a special training at 8 am on July 17 4) Submit Client Counseling (CC) Contact and Public & Media (PAM) Outreach (NPR) forms by the 15 of the month following the counseling session or event through the SHIPtalk website or mail completed paper forms to the SHIIP office in Raleigh; 5) Each SHIIP coordinating site must reach out to 50% of the county's total population for activity events (PAM) and report in SHIPtalk; 6) Each SHIM coordinating site must counsel 5% of their county's Medicare population (CC) and report in SHIPtalk; 7) Represent SHIIP at a minimum of 2 healthfair /senior fair /special events utilizing local certified SHIIP counselors; 8) Conduct a minimum of two (2) presentations - at least one (1) New to Medicare or Medicare 101 presentation to the general public and one (1) Medicare Education presentation to a disability group or potential LIS group in your county; and 9) Participate in 75% of the monthly coordinator conference calls conducted from August 2014 through June 2015. SCOPE OF WORK: (Maximum 2 pages) The Enrichment Center is the headquarters for Lee County Senior Services, Senior Games of Lee County, Veteran's Services, and the County of Lee Transit System. The Enrichment Center is the location for the SHIIP trainings and counseling sessions. Information sessions are held at the Center and in the community regarding the Medicare related choices including Medicare Prescription Drug and the Medicare Advantage Plans that are available to beneficiaries. Free counseling sessions are held at the Center and scheduled through the local SHIIP coordinator. These counseling clinics are by appointment with the SHIIP Coordinator and volunteers are utilized as needed. The allocation will be used to enhance the program through informational materials, travel and training reimbursement, advertising, and recognition of SHIM volunteers. Volunteers will be recognized as part of the annual Volunteer Appreciation Dinner that is held every April. Additional events are planned to promote and educate the public about the upcoming 2014 Annual Enrollment period regarding Medicare Part D. To maintain awareness of the services offered, promotional items, brochures, and other printed materials will be used during outreach opportunities such as Expos, community events, and health fairs. Information explaining the requirements for the Low - Income Subsidy (LIS) will be distributed to members of the community, including faith based and civic organizations, who may be able to assist SHIIP with reaching the remaining Page 10 of 16
