;7 { • PRICE PROPOSAL A. BASE PROPOSAL SHP proposes to provide all medical and dental services as described in our technical proposal to the Lee County Jail. SHP proposes to furnish these services for the annualized price of $175,20 0 the first year with payments made monthly by the tenth day of the month of service. Each monthly installment during the first year would be $14,600. This proposal is based upon an Average Daily Population (ADP) of up to 125 inmates as a monthly average. If the monthly average ADP should rise above 125, a per diem charge of $1.85 per inmate for the number of inmates above the base of 125 will be required (payable monthly) to cover the increased costs in the first year. Our approximate cost breakdown is as follows: Nurse and Physician $89,985 • Admin costs L b M d S 5,211 a , e upplies, O-T-C meds 4,875 Liability Insurance 10,500 Pool Costs (see (B) below) 50,000 Home office overhead, travel, etc. 14,629 Total Price $175,200 B. COST POOL SHP proposes to establish a "Cost Pool" which would account for the costs of all services performed off-site, all x-ray costs, and all prescription medications. This pool would be set at $40,000 and is included in the base price above. SHP would pay all covered costs up to this pool limit. If the costs exceed $40,000 in a single year, then SHP will continue to share in the next $20,000 of cost on a 50/50 basis. If the total pool-related cost should exceed $60,000 in any year, then we would either pay for the services and invoice the cost back to Lee County at 100% of the excess, or would have all billings in excess of $60,000 sent directly to the County for payment to the provider. Our maximum cost would be $50,000, which is $40,000 plus half of the next $20,000_ • We have good experience in North Carolina in controlling pool expenses.
