BK:00026 PG - 0871 <br />programs and services in the community through local churches, news bulletins, physician's offices and <br />banks. Our weekly calendar is printed in the newspaper and special programs and events are also printed. <br />Announcements are made monthly on local radio stations and local public service announcements on <br />radio and television stations are used as advertisement for our programs and services. Our web -site and <br />other social media sites will advertise our programs and services. Staff members present agency <br />information in rural churches and community centers. Senior Services collaborate with other local <br />agencies to provide health and wellness information at low income housing communities and programs <br />are presented to tenants which include the Hispanic population. Many of them are unaware of the services <br />offered at the center. Several staff members plan to take flyers translated into Spanish to all Hispanic <br />businesses and churches. We will seek a contact person at each business and church to communicate our <br />services and programs with and to help us increase this awareness to the Hispanic population. We will <br />have a bilingual older adult accompany us for translation purposes. <br />Certified staff will plan to teach an Evidenced Based Class, Living Healthy with Diabetes at some of our <br />senior apartment complexes. An annual Black History Celebration is held in which volunteers make <br />contacts in communities and local churches to reach minority and low income and rural elderly. Staff <br />participates or provides flyers and information about services and programs at the center. We are invited <br />by minority churches and churches in rural areas during the year to participate in their Health Fairs. We <br />attend as many as possible to provide information regarding our services and programs. The annual <br />Christmas Angle Tree Project has served approximately 150 clients in which 45% or more live in rural <br />areas. The Angel Box includes paper goods, canned goods, clothing, hand -made afghans, severe weather <br />awareness materials, disaster planning and smoke detector information. UNC Chapel Hill School of <br />Nursing in which students are placed at the center providing blood pressure checks and health <br />information. Example: exercise, eating, disease, safety issues. We will continue a "Walking Club" <br />which was started in 2014 by the UNC Nursing students. This will be a regular activity in our Diner's <br />Club programming. They had 30 plus participants and many of who joined them did not participate in any <br />of our other fitness programs. The Sanford Herald is providing free newspapers to the home delivered <br />meal clients via the volunteer who delivers the meal. Most of these clients are in rural areas and are <br />minority and /or low income. Home Town News, a predominantly African American based newspaper is <br />now available at The Enrichment Center and included in the publication is programs and services detail <br />for senior adults. Blood Pressure checks are provided regularly in collaboration with Community Home <br />Care and Hospice at the center. They are checked the 2n Monday of each month. Our annual Boomer <br />Senior and Caregiver Expo is held in August and we provide transportation to our Diner's Club <br />participants to attend this event. This allows them the opportunity to receive information on many <br />services that may be needed and they are not aware of and they can take advantage of the 20 + health <br />screening's that are offered at the Expo. Many participants would not have this opportunity available to <br />them without the collaboration of the center and COLTS transit system of Lee County. <br />TJAAA Addendum Form July 2014 — June 2015 <br />
