The community service provider shall <br />specify how the service needs of low income, low- income (including low income minority elderly), <br />rural elderly and elderly with limited English proficiency will be met through the services identified <br />on the Provider Services Summary (DOA -732). This narrative shall address outreach and service delivery <br />methodologies that will ensure that this target population is adequately served and conform with specific <br />objectives established by the Area Agency on Aging, for providing services to low income minority <br />individuals. Additional pages may be used as necessary. <br />The CenterPost newsletter and other agency materials are distributed to low income housing communities <br />which informs them of services and programs.
