Information regarding the <br />multiple screenings that are available at the EXPO will also be available. <br />Information from SHIIP (combo form) is available on site regarding the <br />preventative benefits that are available through original Medicare. These <br />folders will be given to Medicare beneficiaries upon their initial visit to the <br />Center. <br />7) Monthly reporting forms for contracted activities and expenses incurred are to be <br />submitted to electronically by the 10 of each month for the month prior to Susan Reed <br />at susan.reeda- wakegov.com A final report of expenses incurred is due on or before <br />November 10, 2014. <br />The following Scope of Work should address the ways the Subcontractor will accomplish the <br />above expectations and /or willingness to comply. <br />SCOPE OF WORK: <br />(maximum 2 pages) <br />As part of Lee County Senior Services, The Enrichment Center and its staff are a valuable <br />resource to the county and surrounding area. Events are planned that will assist all members of <br />the community including those with mental disabilities. With this in mind, the Enrichment Center <br />will host an outreach event whose goal will be to reach as many people as possible that may <br />benefit from the "Extra Help" program (also known as the low income subsidy). This group will <br />include mental health professionals, social services representatives and members of the Seniors <br />Health Insurance Information Program (SHIIP). Pertinent information will be distributed to the <br />attendees that will educate and inform of programs that are available to assist all ages of <br />individuals who are suffering from mental and physical disabilities. Additional information will be <br />distributed at the annual Boomer, Senior & Caregiver EXPO that reaches in excess of 2000 people. <br />As the local SHIIP Counseling site is located in our building, a special emphasis will be placed on <br />reaching as many qualifying LIS individuals as possible with referrals being made to the local <br />Social Services Department for additional assistance through the MSP program. Information will <br />be entered into the national SHIPtalk as required by SHIIP by the local coordinator with reporting <br />of individuals and the Public and Media Outreach forms that are a direct result of the outreach <br />sessions being forwarded to the TJAAA office as required by the grant guidelines.
