�3J <br />4.3 A statement of the Proposer's familiarity and experience with FEMA's Public Assistance <br />Program and applicable laws, rules, and regulations. <br />4.4 List of disaster specific experience within the last five (5) years, including the name of <br />each client, a contact person, the size of each project, and response time. Additionally, <br />Proposer must include in this section a statement listing and describing each and every <br />lawsuit in the past five (5) years in which the Proposer sued, or was sued by, any of <br />Proposer's clients. <br />4.5 A debris management and response plan applicable for the scope of work. <br />4.6 Proposer must provide evidence in the form of a letter from a bank or surety licensed to <br />do business in the State of North Carolina confirming Proposer's financial capability to <br />finance a multi - million dollar volume of work for a minimum of 45 days without <br />interference or a slow -down in the work. Proposer must also demonstrate bondability by <br />submitting a letter from its surety stating that Proposer has a bonding capacity of at least <br />$20 million. <br />4.7 References: Include a reference list of at least three (3) clients to whom the Proposer has <br />provided similar services as prime contractor within the past five years. References must <br />include the following information: <br />Name of Client <br />Address <br />Contact Person <br />Title <br />Telephone Number <br />Date(s) of Service <br />Brief Description of Service(s) <br />Cubic Yards Recovered <br />Cubic Yards Reduced <br />Contract Value <br />4.8 Proposers must identify the person who shall serve as authorized negotiator for Proposer <br />should Proposer be selected to negotiate with County. <br />4.9 Proposer must state whether Proposer or any employee thereof anticipated being assigned <br />to provide debris removal services has been a defendant in any proceeding involving or <br />arising out of debris removal services within the past five years. <br />4.10 Proposer must state whether or not Proposer has had a contract related to debris removal <br />canceled within the past seven years.
