4� ckya� j <br />02/15/14 <br />63 1 <br />Please read the Consolidated Agreement carefully and review the list below of substantive changes <br />from last year; there have been a number of formatting and wording changes in the document as well. <br />Summary of Changes to the FY2015 Consolidated Agreement <br />1. A. 10. Includes new language around the inclusion of evidence-based str&Ll#es in <br />Action Plans developed based on the Community Health Assessment (consistent with <br />the recommendations in the NCIOM Report) requiring a plan for staffing, training, <br />implementation and monitoring/evaluation for each evidence -based strategy. <br />2. A. 12. Deleted language re: PHTIN and replaces it with "access to webinars" <br />3. A. 14. Adds language requiring notification of the Local Technical Assistance and <br />Training Branch any time there is a legal name change and the submission of a revised <br />O-chart for any Consolidated Haman Services Agency that includes public health. <br />4. A new B.19. describing the new mechanism for filing for reimbursement for local staff <br />who participate in CIT training. <br />5. A new Business Associate Agreement per the changes in HIPAA requirements and per <br />the agreement at the November, 2013 NCALHD meeting that this would come from <br />DPH to locals. <br />6.
