C 4 8 Attachment 2 • For the period 6/1/2009 - 6/30/2010 Statement of Work Grantee: THE ENRICHMENT CENTER This statement should be a short summary describing what your Grantee does and how the Grantee will use these funds. The uses of these finds are not limited to but MUST include the following activities: I) Hold regular (weekly) counseling clinics during the period 10/2009 through 3/2010, 2) Coordinate a county volunteer recognition event during the period 9/1/2009 - 10/31/2009, 3) Expand Low Income Subsidy (LIS) outreach and enrollment in the county by participating in at least one outreach activity targeted to LIS populations, 4) Attendance at the SHIIP Coordinators' Training Conference on July 29 and 30, 2009 is mandatory for all coordinators. 5) Each county coordinating site must reach out to at least two faith based organizations in the grant period either though outreach, volunteer recruitment, or counseling. All activities of program must be reflected in mid-year and end-of- year reports. SCOPE OF WORK: (Maximum 2 pages) &_he Enrichment Center is the headquarters for Lee County Senior Services, Senior Games of Lee County, eteran's Services, and the County of Lee Transit System. Space is made available for the SI-11111 trainings and counseling sessions. Information sessions are held at the Center and in the community regarding the Medicare related choices including Medicare Prescription Drug and the Medicare Advantage Plans that are available to beneficiaries.-Free counseling sessions are held at the Center and scheduled through the local SHIIP coordinator. The allocation will be used to enhance the program through informational materials, travel and training reimbursement, advertising, and recognition of SHIIP volunteers. Additional events are planted to promote and educate the public about the upcoming 2009 open enrollment period regarding Medicare Part D. To maintain awareness of the services offered, promotional items, brochures, and other printed materials will be used during outreach opportunities such as Expos, community events, flu/H IN] inoculation clinics and health fairs. Information explaining the requirements for the Low-Income Subsidy (LIS) will be distributed to members of the community, including faith based and civic organizations, who may be able to assist SHIIP with reaching the remaining Low-Income Subsidy (LIS) eligible beneficiaries in our county. • Page 8 of 12
