D. <br />_ Information <br />_ Action Item <br />Meeting Date: May 4, 2009 XX Consent Agenda <br />SUBJECT: FY 09/10 Home and Community Care Block Grant <br />DEPARTMENT: Lee County Senior Services <br />CONTACT PERSON: Debbie Davidson, Senior Services Director <br />REQUEST: To approve the Home and Community Care Block Grant for Older <br />Adults County Funding Plan <br />SUMMARY: The total funds allocated from the Home and Community <br />Care Block Grant for FY 09/10 to Lee County Senior Services <br />through the Triangle J Area Agency on Aging are proposed at: <br />$339,776. A 10% match with a required. <br />Funds are proposed to be used in the following services: <br />General and Medical Transportation, Congregate and Home <br />Delivered Meals, Information and Assistance, Senior Center <br />Operations, Minor Home Repair, Health Promotion and <br />Outreach. <br />BUDGET IMPACT: <br />ATTACHMENT(S): County Funding Plan, Lead Agency Designation form, Standard <br />Assurances, Agreement for the provision of services <br />PUBLIC HEARING: No <br />PRIOR BOARD ACTION: N/A <br />Approve County Funding plan as recommended by the Lee County <br />Senior Services Advisory Board <br />Eli? <br />
