C7 <br />1 <br />G~'t <br />® CONTRACT FOR SERVICES PROVIDED FOR <br />UNDER TITLE III-E OF THE OLDER AMERICANS ACT <br />(NC FAMILY CAREGIVER SUPPORT PROGRAM) <br />THIS AGREEMENT, entered into as of this I st day of July 2008, by and between Lee County Senior <br />Services, (hereinafter referred to as the "Agency") and the Triangle J Council of Governments' Area <br />Agency on Aging, (hereinafter referred to as the "Area Agency"). <br />WITNESSETH THAT: <br />WHEREAS, The Area Agency wishes to have performed and the Agency wishes to provide certain <br />aging services in connection with activities financed in part by Older Americans Act grant funds <br />provided to the Area Agency from the United States Department of Health and Human Services <br />through the North Carolina Division of Aging and Adult Services (DAAS). <br />NOW THEREFORE, in consideration of the premises, and mutual covenants and agreements <br />hereinafter contained, the parties hereto agree as follows: <br />1. Employment of the Agency. The Area Agency hereby agrees to engage the Agency and the <br />Agency hereby agrees to perform the services hereinafter set forth in accordance with the terms <br />® and conditions contained herein. <br />2_ Scope of Services. The Agency shall facilitate Family Caregiver Support services to caregivers <br />caring for family individuals who are 60 years of age or older (and also for grandparents and <br />other relatives, age 55+, raising grandchildren eighteen years of age or younger) residing in Lee <br />County. All services are to be performed in accordance with the Agency's annual NC Family <br />Caregiver Support Program (NCFCSP) proposal for services and budget submitted to, and <br />approved by, the Area Agency, the terms of which are incorporated herein by reference. In <br />addition, the Agency agrees to perform services in full compliance with NC Family Caregiver <br />Support Program and Area Agency requirements. <br />3. Time of Performance. The services of the Agency are to commence no later than July 1, 2008 <br />and shall be undertaken and pursued in such sequence as to assure their expeditious completion <br />on or before the end of the contract period, June 30, 2009. <br />4. Contract Administration. The contract administrator for the Area Agency shall be Joan M. <br />Pellettier, Director, Area Agency on Aging. The contract administrator for the Agency shall be <br />Deborah Davidson, Executive Director of Lee County Senior Services. It is understood and <br />agreed that the administrator for the Agency shall represent the Agency in the performance of <br />this contract. The Agency shall notify the Area Agency if the administrator changes during the <br />contract period. <br />11 <br />1 0r8 <br />
