A Change <br />of Custody Form will again be signed by both the LCI truck driver, and the designated LCI <br />Conversion Representative. <br />o LCI's facility has 24/7 security system, monitored by a third party surveillance company. <br />The document conversion division is accessible via keycoded locked doors only to <br />authorized personnel. <br />o All data, once scanned into Document Mall, is stored in the Sawis datacenter, which is <br />SSAE 16 compliant. Savvis offers both physical and internal defenses against <br />unauthorized access. <br />Services excluded from the project scope <br />This project does not cover the following functions or deliverables. <br />• Custom indexing <br />• LCI does not make any decisions regarding whether or not to scan any particular document. All <br />documents, including sticky notes, will be scanned in the order in which they are filed in the <br />paper files. <br />• LCI will not scan and index files that exceed the approved budget /purchase order from the <br />Customer. <br />• Long term storage of paper files. <br />• Data export to discs or external hard drives (is available for an additional fee) (See Data Export) <br />Customer file types included in the project scope <br />The following file type is included in the scope of this project. Any additional file types will require the <br />execution of a Change Order and incur additional costs. <br />File type: Medical Records <br />Services Detail /Project Scope <br />The following are the services and tasks that LCI will provide in fulfillment of the defined deliverables <br />(the "Services ") of the project described in this SOW. LCI shall provide the Services at 2711 Tramway <br />Road, Sanford, North Carolina 27332. Estimated delivery and /or service schedules contained in this SOW <br />are non - binding estimates. <br />Training and Documentation <br />LCI staff will train the Customer's staff with training in accessing, viewing and printing data through the <br />document repository. This is for additional training for new Customer staff, or refresher sessions that <br />may be conducted on an as- needed basis for an additional cost to fees included in this Statement of <br />Work at LCI's standard hourly rates. Initial training for Customer staff was completed May, 2013. <br />Training will include: <br />
